Performance of HPV-DNA testing compared to cytology in cervical cancer screening: an integrative review

Introduction: Cervical cancer remains an important public health problem. Conventional cytology has limited sensitivity, which has driven the adoption of molecular methods such as HPV DNA testing, capable of identifying oncogenic infections at earlier stages. Objective: To analyze the diagnostic performance, applicability, and feasibility of HPV DNA testing compared with cytology in cervical cancer screening within Primary Health Care. Methodology: An integrative review was conducted using the BVS, PubMed/MEDLINE, SciELO, and CAPES databases, with DeCS/MeSH descriptors. Original articles published between 2020 and 2025, available in full text and directly addressing the use of HPV DNA testing in screening, were included. Screening was performed by two independent reviewers using the Rayyan software, resulting in the selection of 15 studies. Results: The studies demonstrated superior diagnostic performance of HPV DNA testing compared with conventional cytology and indicated consistent applicability in Primary Health Care, including high acceptability of supervised self-sampling. Conclusion: HPV DNA testing is a sensitive and feasible method for cervical cancer screening, with the potential to strengthen prevention strategies in Brazil, especially when implemented alongside service reorganization and the training of nursing professionals.
