Comparison between lumbar stabilization exercises and the Pilates method in pain and functional outcomes in individuals with chronic low back pain: an integrative literature review

Abstract

Low back pain is one of the most prevalent musculoskeletal conditions worldwide, being the leading cause of functional disability and significantly impacting quality of life, especially in its chronic form, which has a multifactorial nature. In this context, physiotherapeutic interventions such as lumbar stabilization exercises and the Pilates method have been widely used in the management of this condition. The present study aimed to analyze the effects of these interventions in the treatment of chronic low back pain, with emphasis on outcomes related to pain, functionality, and quality of life. This is an integrative literature review with a qualitative and descriptive approach, guided by the PICO strategy. The search was conducted in the PubMed, PEDro, SciELO, and LILACS databases, including studies published between 2021 and 2026. After the selection process, 7 studies were included in the final sample. The results demonstrated a significant reduction in pain and improvement in functionality, with pain decreasing from 5.5 to 2.7 on the VAS scale in isometric exercises and from 5.8 to 3.7 in isotonic exercises. A 58% reduction in pain was also observed in interventions associated with pain education, compared to 42% in isolated stabilization. The Pilates method showed additional benefits in quality of life and psychosocial factors. It is concluded that both approaches are effective, with no consistent superiority, and that the choice of therapy should be individualized.
