Complex–compound odontome in the left maxilla: surgical treatment

Abstract

Compound-complex odontoma is an uncommon lesion scarcely reported in the literature, presenting characteristics that differ from conventional odontomas. The aim of the present study was to report a clinical case of compound-complex odontoma and its surgical management. An 18-year-old male patient was referred to the Department of Oral and Maxillofacial Surgery with a history of a radiographic finding and delayed eruption of an anterior maxillary tooth. Clinical examination revealed cortical bone expansion and absence of tooth 23. Computed tomography demonstrated a central hyperdense lesion associated with multiple rudimentary tooth-like structures surrounded by a hypodense halo. Based on the main diagnostic hypothesis of odontoma, conservative enucleation and curettage were performed, along with bonding of an orthodontic button to tooth 23 for orthodontic traction. The patient presented satisfactory postoperative healing, without complications or evidence of recurrence. Accurate diagnosis of oral cavity lesions is essential for establishing conservative and effective therapeutic approaches.
